Alternatively, you can download and print a Transfer in Form and fill it in yourself.

Step 2 - Sign and obtain certified identification.
Whether you use our Online Consolidation Form or manually fill it in, you'll need to sign your forms and obtain certified identification for each form.

Further Information

Before you transfer your super, make sure you check with your other fund(s) if they charge any withdrawal or termination fees.  It's also likely that where insurance is held within another fund, that fund will cancel those insurance benefits once you transfer your account balance to another fund, and for some members, they may not be able to get this insurance back within their new fund.  It pays to be sure that's the right move before you send in the form.  If unsure, please speak with your local Member Services Consultant about your options.

Superseeker searches a vast database of lost superannuation accounts for you. All you need is your full name, your TFN, and your date of birth.
