EasyChoice Membership Application Form
For those members currently in EasySuper who would like to transfer to EasyChoice and gain insurance cover with their superannuation

General forms (for use by members of all categories)

Tax File Number (TFN) Notification Form
Members who did not notify us of their tax file number when first joining who subsequently wish to do so should complete this form.

Super Choice Form
This form is used by eligible members to instruct their employers to send their contributions to RecruitmentSuper.
The RecruitmentSuper Letter of Compliance is also included to identify RecruitmentSuper as a complying fund that can accept employer contributions.

Change of Membership Details Form
Used by members who want to change any of their personal details.

Transfer In Form
Used by members who wish to transfer another superannuation account in their name to RecruitmentSuper.

Direct Debit Request Form
Used by members who wish to make Member Voluntary Contributions through Member Online Services.

Voluntary Contributions Form
Used by members to accompany a cheque or to advise of an Electronic Funds Transfer sent to us for a Member Voluntary Contribution.

Nomination of Beneficiary Form
Used by members for nomination of dependants in the case of death.

SuperMatch Search Authorisation Form
Used by members to request a search to find their lost super.

The Benefit Withdrawal Form is used by members who are able to claim their super, have left their employer or wish to transfer their account to another fund. To obtain a copy of this form and proof of identification requirements for your situation, please contact our Customer Service Centre on 1300 304 000.
