Super choice form - Take your RecruitmentSuper with you
Instruct your employer to send their contributions to RecruitmentSuper.
The RecruitmentSuper Letter of Compliance is included. This is required to identify RecruitmentSuper as a complying fund that can accept your employer's contributions.

Tax File Number (TFN) notification form
Tell us your tax file number.

Voluntary contributions form
Tell us about a cheque or Electronic Funds Transfer sent to us for a member voluntary contribution.

Apply for or increase your death and total and permanent disablement insurance cover. Applicable only to SelectPlus members.

Personal statement
If you are applying for death and total and permanent disablement insurance totalling more than $1,000,000, or have answered "Yes" to any of the questions on the Insurance Application Form, you will need to complete this Personal Statement.

*If medical testing or health information is required in relation to a request for insurance, RecruitmentSuper may arrange for you to be contacted by Lifescreen. Lifescreen is a mobile health service provider who can come to your home or office at a time convenient to you to perform medical tests required by the fund's insurer, CommInsure, in order to assess your application for insurance.

Accessing your super

Our Benefit withdrawal form is used to access your super. To obtain a copy of this form and proof of identification requirements for your situation, please contact our Customer Service Centre on 1300 304 000.
